This database contains information on clients placed or waiting to be placed in a Long-Term Care Home. Information includes patient application information and choices for long-term care homes. Data is collected by the Health Shared Services Ontario (HSSOntario) from the 14 Local Health Integration Networks (LHINs) and submitted to the Ministry of Health and Long-Term Care on a monthly basis.
