Information within this dataset is captured through the administration of a bi-annual survey where Local Health Integration Networks (LHINs) provide an assessment of the impact of Convalescent Care Program (CCP) beds on alternate levels of care (ALC) rates, discharges to community, wait-time for long-stay beds placement, local needs and plans to address service gaps.
