These tables include information related to Developmental Services Ontario (DSO) office functions including intake, eligibility determination and assessment. The dataset includes responses to the Application for Developmental Services & Supports (ADSS) and Supports Intensity Scale (SIS).

DSO offices are the primary contact for information about adult developmental services and supports, and the single application point for ministry-funded developmental services and supports.
