IRM is a web-based centralized digital reporting tool used to track inmate / probationer / parolee, employee, and other types of incidents that occur in institutions, while under community supervision, and in corporate settings. Its purpose is to replace paper processes with a computerized system to manage incident reporting and to support internal tracking of trends in incidents. IRM was implemented in all adult correctional institutions on October 11, 2022, and it currently contains Inmate Incident Reports (IIRs) for all inmate-related incidents, including use of force incidents.

IRM is an interactive tool that presents drop-down menus or other opportunities to provide information based on previous selections in the data collection process. When completing an IIR in the IRM, after providing basic information about the date, time, and location of the incident, users will select the appropriate incident type(s) and subtype(s), describe the incident, identify inmates and staff who were involved, et cetera.

There are 15 available incident types for IIRs on IRM: Assault, Threat, Death of an Inmate, Emergency Situation, Escape / Attempted Escape, Lockdown, Major Fire, Use of Force, External Inquiries, High Profile Inmate, Serious Contraband, Serious Injury / Illness / Self Harm, Improper Release / Detention, Unlawfully at Large, and Other.
