Under the authority of Ontario's CFMA, the Ministry of Health and Long-Term Care investigates cases of extra-billing, illegal annual or block fees, and queue jumping for OHIP-insured health services. This dataset contains specific information for each CFMA investigation in order to track and keep a record of current and past investigations. This includes, but is not limited to:

  • patient name
  • health card number
  • provider name
  • facility name
  • description of type of health service(s) received
  • type and amount of unauthorized payment
  • status and details of the investigation
