Restriction: Privacy - Dataset contains personal employee information.

This dataset contains information collected by managers from employees within the Ontario Ministry of Agriculture, Food and Rural Affairs (OMAFRA) who experienced incidents or accidents in the workplace. The collection of this data is required under the Occupational Health and Safety Act (Bill 168).

The data collected is:

  • used to conduct investigations of workplace violence, harassment, or threats of violence
  • shared with the Centre for Employee Health, Safety and Wellness to enable appropriate Workplace Safety and Insurance Board (WSIB) reporting
  • provided to Joint Health and Safety Committees and Health and Safety Representatives to identify, action, and resolve health and safety concerns in ministry buildings

Data includes:

  • employee information
  • incident/accident information (date, time, location)
  • description of the incident/accident
  • corrective action/prevention
