This data is not and will not be made available. The data was reviewed and cannot be released to the public because of reasons outlined by the Digital and Data Directive.

Restriction: Privacy - This dataset contains specifics regarding children's names and other highly private information regarding individuals involved in the occurrences.

This reporting consists of a Regional Office summary of the serious occurrence report provided and a summary of actions taken as a result of the occurrence. The Ministry of Children and Youth Services/Ministry of Community and Social Services 2013 Serious and Enhanced Serious Occurrence Reporting Guidelines require all ministry-funded service providers to report serious occurrences that have occurred involving children.

The dataset contains identifiable client-specific information.
