Integrated Transition Planning for young people with developmental disabilities

Data on the total number of unique individuals who received a copy of their Single Integrated Transition Planning (ITP) since the beginning of the ITP protocol. Data is collected for three Priority Groups:
• Priority 1: Adults at age 18 and over and who are currently receiving children's residential services
• Priority 2: Young people between the ages of 14 and 17 who are currently receiving children's residential services
• Priority 3: Young people at age 14 and over who are receiving non- residential children's services (e.g. respite, counselling and treatment, family support services etc.)
