All public hospitals, inter-disciplinary primary health care organizations, Community Care Access Centres, and long-term care homes are required to submit a Quality Improvement Plan (QIP) annually. QIPs have three components (the progress report, the narrative and the work plan) that describe the quality improvement activities and achievements of the previous year, provide context about the organization, and outline steps for the year ahead.
