Transitional aged youth reporting

Transitional aged youth (TAY) reporting captures data for individuals in the North Region 14 years of age and older who may transfer to adult developmental services. It includes:

  • current service (group home, foster etc.)
  • fiscal and annual costs (where available)
  • confirmation status of adult developmental services eligibility
  • completion status of the Application for Developmental Services and Supports (ADSS) & Support Intensity Scale (SIS)
  • completion status of the Ontario Disability Support Program (ODSP) application

This reporting aids planning/forecasting for transitioning individuals.
