These survey results track field office staff activities in the Service Delivery Model Technology (SDMT) system, including: client inquiry, preliminary assessment, client appointment, client interview and application, eligibility determination, Disability Adjudication Unit (DAU) referral, grant social assistance, participation requirements, financial processing, ongoing case management, financial eligibility review, appeals, and ODSP employment support.

SDMT is the system that, until October 2014, supported the delivery of social assistance under the ODSP and Ontario Works programs.
